Description

Jext is a treatment used to treat anaphylactic shock in patients with severe allergies. It is a prefilled pen, containing adrenaline, which is injected into the muscular tissue in the thigh. During an anaphylactic episode, this works by preventing blood pressure from dropping to dangerous levels. Jext is manufactured by ALK and is only available to buy in the UK with a prescription. It contains the same ingredient as EpiPen and works in the same way.

Because two pens may be needed to treat a reaction in some cases, patients with anaphylactic allergies are advised to carry two with them. These should be administered 10-15 minutes apart, where required.

The term allergy is one which spans a wide range of reactions, from those displaying relatively mild or gradual symptoms, to severe symptoms caused by food or an insect sting. Many allergies, such as hay fever and contact dermatitis, can cause considerable discomfort and irritation, but are often not life-threatening.

Anaphylaxis, however, is a medical emergency. Reactions of this type tend to have a very sudden nature of onset, and are typically caused through contact with certain foods (nuts and shellfish are common triggers) or an insect sting (such as from a wasp). Medications can also sometimes cause this effect.

Symptoms of an anaphylactic response are numerous and might include the appearance of an itchy rash, swelling of the eyes, lips or throat, or loss of consciousness. In severe cases, the airways can swell to a degree which causes an obstruction and breathing difficulties, and a drop in blood pressure may also cause heart failure.

This type of response is the result of the immune system reacting to certain stimuli which it perceives as a threat. In an effort to protect the body from the allergen, the body releases a chemical which has an inflammatory effect, and causes blood vessels to significantly widen. Blood pressure falls dramatically as a result and tissue swelling occurs.

The active ingredient in Jext is epinephrine (adrenaline), which acts on alpha and beta adrenergic receptors. These receptors are connected to blood vessels, and by stimulating them, adrenaline causes blood vessels to narrow, increasing blood pressure and cardiac blood flow. It also helps to reduce muscular airway constriction, making breathing easier.

Directions

Make sure you read the instructions provided in the patient information leaflet carefully when you receive this product. If you have been advised to carry an adrenaline pen, a doctor or nurse should have shown you how to use it. If you haven’t been shown, contact your GP to arrange this immediately. If you are unsure of anything relating to this medicine, speak to your doctor.

  • Use as directed.
  • One pen (300mg) contains a single adult dose.
  • The pen should be used as soon as you notice signs of a severe allergic reaction.
  • Following contact with an allergen, quickly remove the offending allergen from the body if possible; this might be taking a wasp sting out of the skin, or removing a piece of food causing the reaction from the mouth.
  • To use the pen, remove the yellow cap.
  • It can be injected directly into the skin or through clothing, but most only be used on the outer thigh.
  • Position the black tip on the outer thigh. It should be held at a 90 degree angle.
  • Press the tip firmly into the thigh, until you hear a click. Hold the pen in place firmly for ten seconds.
  • Remove the pen, and massage the injection site for ten seconds.
  • After using the pen, call 999 right away.
  • If symptoms persist, you may be able to administer a second dose after 5-15 minutes if your doctor has deemed this course of action suitable beforehand. You will need to use a second injector pen for this as each pen only contains a single dose.

Side effects

It is important when using any medicine to be aware of the potential side effects it may cause.

Recorded side effects associated with the use of Jext include: irregular heartbeat, high blood pressure, chest pain, narrowing of the blood vessels, sweating, nausea, vomiting, difficulty breathing, headache, dizziness, weakness, tremor, anxiety, hallucinations, fainting, changes in blood sugar levels, decreased blood potassium levels, harmful collection of acid in the body, and heart problems.

The leaflet contains a more detailed list of side effects as well as advice on what to do if the pen is injected accidentally into the hand or foot.

Taking it with other medicines

Your doctor needs to know if you use any other treatments, including the following: antidepressants such as tricyclic antidepressants or monoamine oxidase inhibitors; medicines for Parkinson's disease such as COMT inhibitors; treatments which increase sensitivity to arrhythmias such as digitalis or quinidine; or alpha or beta blockers.

Conditions to look out for

Inform your doctor during consultation if you have or have ever had: heart disease; an overactive thyroid; high blood pressure; diabetes; a tumour on the adrenal gland; glaucoma; renal or prostate disease; or low potassium levels or high calcium levels in your blood.

Pregnancy

You should still use your adrenaline pen if you experience an anaphylactic reaction during pregnancy, as your and your child’s life may be at risk.

Driving and using machinery

You should not drive or operate machines after using this pen, and call 999 immediately.

Q&A

Can I still drink alcohol?

If you have an anaphylactic allergy, it is generally advisable not to drink too much alcohol if you are in an environment where allergens could appear and present a risk, as this may hinder your capacity to use the pen.

Following use, alcohol should be avoided, and medical attention should be sought.

Will I still be able to drive?

It will not be safe to drive following use or during a severe allergic reaction. Call an ambulance and do not drive until you have been told by your doctor that it is safe to do so.

Can I take the medicine while pregnant?

Inform your doctor during consultation if you are pregnant or breastfeeding.

If you are pregnant, you should still use this pen to treat anaphylaxis to protect both yours and your child’s life.

How should I store it?

Make sure you keep Jext in a safe environment below 25°C and out of the reach of children. It should not be refrigerated or frozen.

The fluid in the cartridge should be carefully checked on a regular basis. The liquid inside should be clear and colourless. If it is not, the pen should be replaced.
