Fortacin spray is a topical medication used in premature ejaculation. It is made in the UK by Pharmaserve and marketed by Recordati. The active constituents are lidocaine and prilocaine, which are anaesthetic agents. These help to reduce sensitivity in the penis, helping the user to engage in sexual intercourse for longer.
Premature ejaculation is not a straightforward condition to define, as sexual expectations can vary from one couple to another; what constitutes ‘premature’ for one man may not necessarily for another. The clinical definition of premature ejaculation is that which occurs within two minutes or less of penetration commencing, on a persistent basis.
It’s likely that most men will experience the condition in some form or another throughout their sexually active lives; in many cases it might only be occasional or temporary.
For example, many men may feel pressure to perform during the early stages of their sexual relationship with someone, and this can result in premature ejaculation. This anxiety will normally subside as the man’s confidence grows, and premature ejaculation will occur less.
However for other men, the problem may be a chronic one and persist, and they may need to seek help from a healthcare professional to overcome it.
To begin with a doctor will normally advise self-help approaches, such as the squeeze method, to help train the man to last for longer. In the squeeze technique, the man’s partner masturbates them almost to the point of climax, before stopping and gently squeezing the head of the penis for 20 seconds. After 30 seconds, masurbation continues, and then this process is followed again several times.
Other ways of prolonging sex include wearing thicker, extra safe condoms to help lower sensitivity in the penis, masturbating a few hours prior to sex, or taking a deep breath right before the point of ejaculation (this helps to stifle the body’s ejacluatory reflex).
In cases where the above approaches haven’t been successful, a doctor might recommend treatment in the form of a topical anaesthetic. Fortacin spray contains two ingredients; lidocaine and prilocaine. These work by reducing sensitivity at the site of application, and this helps to prevent the transmission of pleasure signals to the brain; enabling the man to last for longer.

Use according to the instructions given by your doctor or pharmacist. Make sure you read the patient information leaflet prior to use, and follow the directions carefully.
- The typical dose is 3 sprays applied to the head of the penis before sex.
- Before use, shake the bottle. Prime the pump by spraying it onto the air three times. Make sure you do this away from your face.
- Peel back the foreskin to expose the head of the penis.
- Apply one dose, holding the bottle upright. Each spray should cover a third of the head of the penis.
- Allow five minutes for the medication to be absorbed.
- After five minutes, wipe off any excess spray with a clean tissue.
- We recommend that you use a condom so that the spray does not come into contact with your partner.
- Leave at least four hours between doses. Do not apply more than three doses in any one 24 hour period.
- Do not exceed the dose prescribed by your doctor.
- If you accidentally spray too much, wipe it off with a clean tissue.

Common (1 in 10 people or less):
In males: erection problems, reduced feeling or burning sensation in and around the penis.
In female partners: burning sensation or reduced feeling in and around the vagina
Uncommon (1 in 100 people or less):
In males: headache, throat irritation, skin irritation, redness on and around the penis, failure to ejaculate during sex, abnormal orgasm, tingling, itching, pain or discomfort in and around the penis, or a high temperature.
In female partners: headache, throat irritation, vaginal thrush, discomfort in the anus and rectum, loss of feeling in the mouth, difficulty or pain passing urine, vaginal pain, or itching or discomfort in the vagina.

Taking it with other medicines
Let your prescriber know if you are taking any other treatments. This is very important if you are using, or have recently used: other local anaesthetics; heart medicines; high blood pressure medicines (such as beta blockers); or treatments to reduce stomach acid (cimetidine).
People taking the following treatments may be at higher risk of methaemoglobinaemia when using Fortacin spray, so make sure you tell your doctor if you are using: benzocaine; chloroquine, pamaquine, primaquine, or quinine; metoclopramide; glyceryl trinitrate (GTN, nitroglycerin), isosorbide mononitrate, or erythrityl tetranitrate; pentaerythritol tetranitrate or other nitrates for angina; sodium nitroprusside or isosorbide dinitrate; nitrofurantoin; sulphonamides; sulfasalazine; dapsone; phenobarbital or phenytoin; or para-aminosalicylic acid (PAS).
Your doctor also needs to be made aware if you regularly come into contact with dyes, such as aniline dyes, or chemical pesticides such as naphthalene.
Please note that the effectiveness of condoms made from polyurethane-based material may be reduced when used in conjunction with Fortacin spray.
Conditions to look out for
Do not use this spray if you or your partner are allergic to anything in it, or if either of you have had an allergic reaction to similar anaesthetics in the past.
You should advise your doctor if you or your partner have, or have ever had, any of the following conditions, as you may need to take special precautions during use: a genetic disease or other condition affecting red blood cells (such as glucose-6-phosphate deficiency, anaemia or methaemoglobinaemia); any medicine sensitivities; or severe liver problems.
Pregnancy and breastfeeding
Fortacin spray is a premature ejacualtion treatment for men, and should not be used by women.
Men should wear a condom while using Fortacin Spray if their partner is pregnant, to help prevent their partner from coming into contact with it.
Fortacin Spray can be used by men whose partner is breastfeeding.
Driving and machinery
It is not likely that this treatment will affect the user’s capacity to drive or use machines.
Can I still drink alcohol?
Fortacin is not thought to interact with alcohol, but drinking alcohol to excess can affect sexual performance, so is not recommended.
Will I still be able to drive?
Your ability to drive should not be adversely affected by the use of Fortacin Spray.
