Product information

Detrusitol is a treatment for urinary urgency, made by Pfizer. It is available in two forms: as a regular film-coated tablet; and as an XL capsule. The latter of these contains a higher dose, but is designed to be taken less frequently, as its contents are dispersed over a prolonged period. The active constituent is a compound called tolterodine, which is an antimuscarinic, and works by relaxing the detrusor muscle.

Urinary urgency is a condition which is characterised by the sudden and irrepressible need to urinate. For some, this may result in passing an amount of urine before being able to reach the toilet. The main cause of it is usually an overactive bladder. Irregular muscular contractions or tension in the urinary tract area may cause increased pressure, and the need to urinate urgently or with increased frequency.

Stress incontinence occurs when the bladder leaks after a slight sudden extra pressure such as sneezing and laughing. (For this pelvic floor exercises can often help; your doctor will be able to provide more guidance on these.)

There are a number of reasons why someone might get urinary urgency. Among them are: lifestyle factors, for instance if somebody consumes a high amount of alcohol or caffeine, or doesn’t drink enough water; the taking of medications which affect the production of urine; or the presence of an underlying medical condition, such as a urinary tract infection. Sometimes a person may also develop the condition after pregnancy, or in later life.

If you are experiencing urinary urgency, it is important not to ignore it, as it may be indicative of another condition requiring treatment; particularly so if you notice other symptoms, such as pain or burning when passing urine, or worsening back or leg pain. Medication can help to alleviate symptoms. Detrusitol is an effective drug for urinary urgency, but it is only available to buy in the UK with a prescription from a doctor.

This drug functions by interfering with the transmission of certain signals which pass between the muscle walls around the bladder, and the brain. These signals can sometimes result in the unpredictable bladder contractions which cause urinary urgency. By blocking the passage of these signals, the active ingredient in Detrusitol, tolterodine, helps to reduce symptoms.

Follow the directions issued by your prescriber when applying this treatment, and read the guidelines in the patient information leaflet fully before you use it. This will help you to reduce the likelihood of side effects and make sure the drug is as effective as possible.

  1. Take as directed.
  2. For Detrusitol standard tablets, this may be one 2mg dose, twice per day, for a period determined by your prescriber.
  3. Your doctor may reduce this to 1mg if you experience any mild side effects.
  4. For Detrusitol XL capsules, the dose may be one 4mg tablet, taken once a day, for a specified duration.
  5. Again, a practitioner may decide to switch your dose to standard tablets in the event that signs of sensitivity are displayed.
  6. Swallow tablets or capsules whole with water.
  7. Do not exceed the dose prescribed for you.
  8. Never compensate for a missed dose with a double dose. Take it as soon as you remember if you forget, unless it is time for your next dose. In which case, simply carry on the course as normal, and if you are concerned, speak to your doctor.
  9. Review may be carried out after a period of two to three months of use.

Side effects & Warnings

Side effects

It is important to be aware of the potential reactions a drug may cause, as those more serious instances may require medical attention. Tell your doctor immediately if you notice anything resembling symptoms of an allergic reaction. More information on what these are can be found in the patient information leaflet.

Very common (1 in 10 patients or more):

Dry mouth, headache.

Common (1 in 10 patients or less):

Bronchitis, dizziness, sleepiness, pins and needles, dry eyes, blurred vision, vertigo, palpitations, indigestion, constipation, abdominal pain, feeling or being sick, dry skin, pain when passing urine, drowsiness, chest pain, water retention, increase in weight, or diarrhoea.

Uncommon (1 in 100 patients or less):

Nervousness, increased or irregular heartbeat, heart problems or memory loss.

Taking it with other medicines

Tell your prescriber about any other drugs you are currently using. Detrusitol may interact and be unsuitable for use in conjunction with the following: some antibiotics; antifungals; treatments for HIV; metoclopramide, cisapride or similar products; drugs for an irregular heartbeat such as amiodarone, sotalol, quinidine, or procainamide; other antimuscarinics.

Conditions to look out for

It is not suitable for those with: urinary retention; narrow-angle glaucoma; myasthenia gravis; severe ulcerative colitis; or toxic megacolon. Those with the following the conditions may be required to use this product with caution: difficulty passing urine; gastrointestinal disease; liver or kidney problems; neuronal disorders; hiatal hernia; severe constipation; irregular heartbeat or heart disease; or abnormally low levels of potassium, calcium or magnesium in the blood.

Pregnancy and breastfeeding

Detrusitol should not be taken during pregnancy or breastfeeding. If you are planning to become pregnant, speak to your doctor before taking this medication.

Driving and using machinery

This treatment may cause side effects such as dizziness and sight problems. If you are affected, do not drive or operate machines.

Can I still drink alcohol?

Your treatment should not directly be affected by the consumption of alcohol, however it should be noted that drinking large amounts can exacerbate symptoms of urinary urgency.

Will I still be able to drive?

This drug may cause dizziness, therefore it is vital to refrain from driving if this treatment makes it dangerous for you to do so, and speak to a doctor as soon as possible.

Can I take the medicine while pregnant?

It is not suitable for women who are pregnant or breastfeeding.

How should I store it?

In a safe place, below 30°C.

Am I allergic to anything in the medicine?

The tablets contain: tolterodine tartrate, microcrystalline cellulose, calcium hydrogen phosphate dihydrate, sodium starch glycollate, magnesium stearate and colloidal anhydrous silica, hypromellose, microcrystalline cellulose, stearic acid and titanium dioxide (E171).

In the XL capsules are the following ingredients: tolterodine tartrate, sugar spheres, hypromellose, surelease E-7-19010 which contains ethylcellulose, medium chain triglycerides and oleic acid, gelatine, indigo carmine (E132), titanium dioxide (E171), shellac glaze, propylene glycol and simeticone.

Do not use these products if you are allergic to anything in them.

Is it available over-the-counter?

You can only get Detrusitol or Detrusitol XL on prescription in the UK. This is because a doctor will need to check if it safe for you before use.
