Kliofem is a tablet made by Novo Nordisk. It contains synthetic versions of oestrogen and progesterone, and is used in hormone replacement therapy. The medication treats symptoms associated with menopause in cases where a person has not had their womb removed during a hysterectomy (non-surgically induced menopause). This pill is taken once a day, at the same time each day. The drugs are dispensed by a calendar pack, which is a device designed to help the user stay on track with their course.

Most women will go through the menopause when they reach their late 40s or early 50s. It is characterised by the slowing and eventual stopping of menstruation, and the cessation of ovulation (the process by which the ovaries produce an egg). In relation to these transformations in the reproductive system, the amount of hormones the body naturally produces also changes. Oestrogen levels fall, and as a result, certain symptoms can occur.

These tend to vary from person to person, but among the most common effects of the menopause are headaches, vaginal dryness or irritation, night sweats or hot flushes, problems sleeping and mood swings. Additionally, women who are postmenopausal are thought to be more prone to osteoporosis.

These symptoms will not affect everyone to a problematic extent. In fact, many women going through the menopause may only experience them to a mild degree. But in other cases, these effects might manifest in a form sufficient enough to be disruptive to day-to-day life, and warrant medical attention.

HRT, or hormone replacement therapy, is the treatment path commonly issued in these instances. It aims to remedy oestrogen-deficiency by supplementing decreased levels of the hormone with a synthetic alternative; thus alleviating symptoms. There are several different types of HRT. Kliofem is a continuous, combined form. This is most often issued in cases where the person receiving treatment has not had their womb surgically removed, and has had at least one year since their last menstrual bleed.

As well as containing an oestrogen (estradiol), combined HRT drugs such as this one also contain a progestogen (in this case norethisterone). This is because high amounts of oestrogen can cause the endometrium to thicken to a point where it may pose an increased risk of developing cancer. Norethisterone works as a countering agent, keeping oestrogen levels under control and maintaining a hormonal balance.

Always make sure you read the instructions provided with your medication before you use it, and adhere to the directions specified by your prescriber. Doing so will help to maximise the efficacy of the drug and reduce the occurrence of side effects.

  1. Take as directed by your prescriber.
  2. This will be one pill, taken at the same time each day.
  3. Each tablet comes preloaded in a disk called a calendar pack. This is designed to help you keep track of your treatment. Detailed information on how to use it is supplied in the leaflet.
  4. Swallow your pill whole with water.
  5. Never exceed the prescribed dose.
  6. If you miss a dose, try to take it when you remember, and within 12 hours. If this is not possible and your next administration is due, skip the missed dose and continue the course as before.
  7. Do not make up for a forgotten dose by doubling up.

Side effects & Warnings

Side effects

Despite the fact that not everyone who uses this drug will get them, it is important to be aware of the side effects this product may cause. If you experience any harmful side effects or any signs of the following, consult your doctor right away: breast, cervical or ovarian cancer; an allergic reaction; a heart attack or stroke; angina; a blood clot; or liver failure. You can find more detailed information on what these signs look like in the leaflet provided.

Very common (1 in 10 people or more):

Breast pain or tenderness, or vaginal bleeding.

Common (1 in 10 people or less):

Headache, vaginal inflammation, fluid retention, weight gain, migraine, fungal infection, depression, feeling sick, stomach pain, swollen breasts, back pain, cramping, fibroids in the uterus, or swelling in the arms and legs.

Uncommon (1 in 100 people or less):

Flatulence, acne, hair loss, itching or hives, inflammation of a vein, or anxiety.

Rare (1 in 1,000 people or less):

Blood clots in the legs or lungs.

Taking it with other medicines

Your prescriber needs to be informed during consultation if you are using any other treatments of any kind. Kliofem may interfere with the function of, or be rendered less effective by, the following: drugs for epilepsy including phenobarbital, phenytoin and carbamazepine; the tuberculosis medicines rifampicin and rifabutin; antivirals for HIV such as nevirapine, efavirenz, ritonavir and nelfinavir; St John’s Wort; ketoconazole; or cyclosporine.

Conditions to look out for

Refrain from using this item if you have or have ever had: breast cancer or any type of cancer which is responsive to oestrogen; endometrial hyperplasia; any episode of vaginal bleeding for which your doctor could not find a reason; a blood clot or any related condition, such as a heart attack or stroke, or any condition which interferes with blood clotting; porphyria; or liver disease from which your liver function has not yet fully recovered.

Your prescriber may choose to only issue this treatment with special instructions, or not issue it at all, if you have a history of any of the following: fibroids inside your womb; endometriosis; any condition or risk factor which increases the likelihood of a blood clot or the development of an oestrogen-sensitive cancer; high blood pressure; liver disorder; diabetes; gallstones; migraine; systemic lupus erythematosus; epilepsy; asthma; otosclerosis; high level of fat in the blood; fluid retention; or lactose intolerance.

Regular check-ups

You will need to see your doctor in person on a regular basis (typically at least once annually) to monitor how well this medication is working.

Pregnancy and breastfeeding

This item is for use in postmenopausal women only. It should not be used during pregnancy or breastfeeding. If you become pregnant while taking it, stop taking it and let your doctor know immediately.

Driving and using machines

This treatment is not likely to influence your capacity to drive or use machines.

Lactose intolerance

Kliofem contains lactose. If you have an intolerance to some sugars, tell your prescriber.

Can I still drink alcohol?

Yes. It is not thought that alcohol will hinder the activity of this drug.

Will I still be able to drive?

Yes. It is unlikely that your ability to drive will be affected. However, you should not drive if you experience any side effects which might make doing so unsafe, and notify your doctor.

Can I take the medicine while pregnant?

It is not suitable for women who are pregnant, think they might be pregnant, or are breastfeeding.

How should I store it?

Keep it in a safe place, at room temperature.

Am I allergic to anything in the medicine?

This drug contains the following: estradiol, norethisterone acetate, lactose monohydrate, maize starch, hydroxypropyl cellulose, talc and magnesium stearate. Do not use it if you allergic to any of these ingredients.
